Office visit - $100
Physical - $150

*$25.00 is additional charge for the evening hours office visit
(Monday- Friday: 5:00 pm to 7:00 pm, Saturday: 9:00 am to 3:00 pm)

Note:
There may be additional charges for additional time with the doctor, lab work, injections, x-rays, procedures, surgeries, etc. Please note that $100.00 may not be the total charge for the visit.
